Instructions on applying for the senior exemption
1) Fill in dates of birth; include the name and birth date of your spouse or joint owner
even if he/she is not yet 65 years of age.
2) Fill in phone number.
3) Attach a copy of proof of age and residence such as a state ID card or drivers
license. A medicare card cannot be used.
4) If the property is in trust, attach a copy of the trust agreement or a letter from your
bank or attorney stating that you have a primary beneficial interest in the trust.
5) Sign and date the form.
6) Mail your application and photocopies to the Supervisor of Assessments 2200 N
